COVID-19 Related Accommodation Requests

During the COVID-19 pandemic, the University of Notre Dame remains committed to offering an unsurpassed undergraduate education that nurtures the mind, body and spirit.  The University also seeks to advance human understanding through scholarship, research and post-baccalaureate programs that heal, unify and enlighten.

Undergraduate and graduate students, including those in professional schools, with a documented health condition that places them at high risk, according to CDC COVID-19 guidelines, or who have other special circumstances may seek reasonable accommodations or other modifications by engaging with Sara Bea Accessibility Services.
The forms below are for all Covid-19 related requests. Please include as much information as possible in the request. Following receipt of the request, a staff member will contact you. 
The University will provide Covid-19 Testing to any student who wants it prior to their departure at the end of the semester. The testing is not mandatory. We want to support in particular those students who are going home to immediate households with an individual at high risk for COVID-19 complications. These students will receive priority testing, as well as those who have approved health accommodations. Use the forms below to request priority testing. 

New to Sara Bea?

If you have never requested an accommodation before, please complete the information on our Initial Accommodation Request Form.  A Sara Bea Staff member will the contact you to discuss your request in more detail.

Already registered with Sara Bea?

If you are already registered with Sara Bea Accessibility Services or requested a one-time accommodation such as air-conditioned housing prior to your freshman year, please complete our Supplemental Request Form.  A Sara Bea Staff member will then contact you to discuss your request in more detail.

International Student?

If you are an international student and are not able to travel to campus or cannot take advantage of Notre Dame’s partner programs at universities abroad and if you wish to remain engaged academically with the University for the fall 2020 semester, please complete the Academic Continuity Request form to begin exploring that process. Please note that students cannot maintain F-1 status with this option.
